Nursing supports recovery after hospital treatment. The referral and clinical information guide the proposed care.
Rehabilitation
Rehabilitation helps a person work towards greater independence after surgery, illness or injury. The treating team and care centre agree what support is suitable and available.
The next care setting
Discuss the support needed after recovery, including care at home or a centre-based arrangement.
Arrange the referral before admission.
Clinical information
Ask the hospital or treating team to share the referral and clinical information with the relevant Medwell centre. The centre explains the assessment and documents needed.
Care and funding
Confirm suitability, the proposed care and funding authorisation or private payment. A scheme’s authorisation does not guarantee payment.
Planning after recovery
Agree the next care setting, follow-up arrangements and any help needed at home with the treating team and family.
