Medication management.
Help with a medicine routine.
Taking several medicines can make a daily routine hard to manage. Medication support starts with an accurate list of what has been prescribed, the dose and when each medicine is taken.

One clear medicine list
A current list records each medicine, dose and time. The list helps residents, families and clinicians see what is being taken, particularly when a person has several long-term conditions or sees more than one prescriber.
Help with timing
Some people need a reminder or a labelled pill case. Where medicine must be administered by staff, the care plan sets out who does it and when. The level of support depends on the person's ability and the care setting.
The pharmacy's role
A pharmacy may prepare blister packs for a person who self-administers or receives help from a family member. A pharmacist or prescriber must review questions about doses, interactions and side effects; packing alone does not replace that review.
Discuss the arrangement
Ask Medwell's care team to review the person's current medicine list and explain which reminders, staff support or pharmacy packing can be arranged in the relevant care setting. When a prescriber changes treatment, the medicine list and care plan need updating.
GOOD TO KNOW
What to discuss
- Is there a current medicine list?
- Who reminds, prepares or administers each medicine?
- Who updates the list when treatment changes?
Medwell can confirm the care and local arrangements after discussing the person’s needs.
Talk through the next step
Tell Medwell where care is needed and what has changed. The team can discuss the service and what can be arranged.
